I recently had a conversation on the Homeopathy247 podcast about migraines and chronic headaches… and one thing became very clear.
Migraine vs tension headache isn’t always as straightforward as it sounds… even though they’re often treated like they are.
It’s when you actually sit with someone and listen… the differences matter.
Is it pounding, throbbing, and takes you out for the day?
Or is it that constant band of pressure across your shoulders and neck that never quite lets up?
Both are real.
Both are exhausting.
And both are your body trying to tell you something.

What is a migraine… really?
Migraines are considered a neurological condition.
That means it’s not just head pain… it’s your nervous system involved.
People will often describe:
- Throbbing or pulsating pain
- Often one-sided (right of left)
- Nausea or vomiting
- Sensitivity to light, sound, or smells
- Wanting to lie down in a dark, quiet room
- Aura…visual changes like flashing lights or blind spots
- Dizziness or vertigo
They can last anywhere from a few hours to a few days… and when they hit, normal life usually stops.
This is very different from the kind of headache someone can just “push through.”
What is a tension headache?
Tension headaches are the most common type of headache.
They’re often called stress headaches or muscle contraction headaches… and that description actually fits.
You’ll hear:
- Dull, aching pain
- Tightness or pressure across the forehead or back of the head
- Neck and should tension
- Pain that builds through the day
- Triggered by stress, posture, fatigue or missed meals
Most people can still function…
but they’re carrying it all day.
A simple way I explain it:
Migraines stop you.
Tension headaches follow you.

What about sinus headaches?
These are often confused with migraines.
Sinus headaches tend to feel like:
- Pressure in the face, forehead, or teeth
- Congestion or runny nose
- Worse when bending forward
That congestion piece is key.
How homeopathy sees things differently
In conventional care, we often focus on the label or diagnosis.
In homeopathy, we listen for the pattern.

Some migraines follow deeper patterns, especially around hormones or long-term stress.
Sepia
Hormonal or cyclical migraines
Often tied to depletion, over-responsibility
Better from movement or being alone
Irritability or emotional withdrawal
Tension headaches
Calcarea phosphorica
Dull, aching pain
Often from fatigue, growth, or depletion
Neck and shoulder tension
Worse with exertion or long days
Staphysagria
Headaches from suppressed emotion
Holding things in… not saying what needs to be said
Tension builds quietly over time
Often tied to stress, resentment, or over-control
Nux vomica
Driven, high-stress personality
Headaches from overwork, lack of sleep, stimulants
Irritable, sensitive, “pushing through”
If your headaches come with congestion or sinus pressure…. the pattern may be slightly different.

Kali bichromicum
Pressure at the root of the nose or forehead
Thick, stringy mucus
Pain often in a small, specific spot
Pulsatilla
Congestion with thick, bland discharge
Worse in warm, stuffy rooms
Better in fresh air
Often softer, more emotional presentation
Hepar sulphuris
Sensitive, painful sinuses
Worse from cold air
